If you are interested in becoming a new patient, please call our office at (413) 239-7111 to register and schedule your new patient appointment. You may also request an appointment online and our office will reach out to you. You may find a list of accepted insurances on our New Patient: Insurance page.

In order to register you, we require your demographic information and insurance information prior to your appointment. If you will be using insurance, your insurance information must be provided and verified at the time of registration, with no exceptions. Please make sure to call the number on the back of your insurance card to verify that our office is in network. It is the patient’s responsibility to know what their insurance covers and where it is accepted.

Important Notes:
– New patient appointments are not the same as an annual physical. Annual physicals are for established patients only.

– If you have an HMO plan, you are required by your insurance to call them, assign Dr. Rastegar as your PCP, and obtain a reference number prior to your new patient appointment. This is paramount for insurance to cover your bill. Please provide us with the reference number once you’ve obtained it.

– If you change your insurance plan, it is your responsibility to notify us and provide your new insurance information. Failure to notify us of any changes to your insurance can result in your bill being denied and charged directly to you.

Remember to bring a list of any medications you may currently be taking as well as any previous medical records you may want to provide.
