Please call the number on the back of your insurance card directly to verify if your specific plan is in network with our office. We do not know what plans are in network even if we accept the carrier. It is the patient’s responsibility to know what their insurance covers and where it is accepted.
If you have an HMO plan, you are required by your insurance to call them, assign Dr. Rastegar as your PCP, and obtain a reference number prior to your new patient appointment. This is paramount for insurance to cover your bill.
If you change your insurance plan, it is your responsibility to notify us and provide your new insurance information. Failure to notify us of any changes to your insurance can result in your bill being denied and charged directly to you.
Important Note: We DO NOT accept any Medicaid/MassHealth plans. If you have a MassHealth plan through one of our accepted carriers, please call the number on the back of your insurance card to verify that we can accept your plan.
